Note:
- UCSI University Senate, on 22 Mac 2017, has approved that the pre-admission interview and chemistry quiz for BPharm (Hons) to be abolished. Eligible candidate shall be admitted into the programme after approval by the Faculty.
- International students are required to attend for pre-admission interview.
- Students who sit for January exam to apply with actual results in March.

Candidates are advised to bring along original and two (2) copies of certified photocopies of following documents (1 for Registrar's Office, 1 for Faculty):
- Academic documents (Completion letter or Certificate and official full transcripts for Foundation in Sciences, STPM, Alevels, SAM, CPU, SPM, UEC, O-levels etc.)
- English Proficiency Test Results (MUET, IELTS or TOEFL)
- School Leaving Certificate (“Sijil Berhenti Sekolah”)

- MyKad (Malaysians only) or Passport (Non-Malaysians only)
